PLEASE READ THESE TERMS AND CONDITIONS CAREFULLY BEFORE USING THIS WEBSITE OR BOOKING SERVICES
Welcome to the website and telehealth booking portal of Clarion Family Health PLLC (“Practice,” “we,” “us,” or “our”). By accessing or using our website (https://clarion-family-health.com), registering for the PracticeQ Patient Portal, or booking a virtual appointment, you (“Patient,” “User,” or “you”) agree to be bound by these Terms and Conditions.
If you do not agree to these terms, you must not access or use our services.
- Medical Emergency Disclaimer (Not for Emergencies)
IF YOU ARE EXPERIENCING A MEDICAL EMERGENCY, CALL 911 IMMEDIATELY OR GO TO THE NEAREST HOSPITAL EMERGENCY ROOM.
Our virtual services, patient portal, and website messaging systems are NOT intended for urgent or emergency medical situations. Clarion Family Health PLLC does not provide emergency medical services. Message responses via PracticeQ or telephone are monitored only during standard business hours.
- Telemedicine Services & State Licensure
Scope of Virtual Care: Clarion Family Health PLLC provides synchronous (video) and asynchronous (messaging/forms) virtual primary care, routine consultations, chronic care management, wellness evaluations, and diagnostic lab ordering.
Geographic Requirement: Telehealth consultations are offered only to individuals physically located within the State of Florida (or states where our healthcare providers hold active medical licensure or official out-of-state telehealth provider registration) at the exact time of the virtual visit.
Standard of Care: Telemedicine encounters carry the same standard of care as in-person medical evaluations. However, if your clinician determines that your condition requires a physical exam or hands-on procedure, you agree to seek in-person care from a local medical provider or urgent care facility.
- Direct Cash-Pay Financial Policy & Payments
Clarion Family Health PLLC operates on a direct cash-pay practice model.
No Insurance Billing: We do not bill health insurance plans, Medicare, or Medicaid. You are directly responsible for all service fees, consultation charges, and lab fees.
Upfront Payment Collection: Payment for virtual appointments and lab bundles is collected at the time of scheduling via our secure payment gateway (PracticeQ / Stripe).
Diagnostic Lab Fees: Lab orders routed to diagnostic partners (Quest Diagnostics, Labcorp, Fullscript Labs) under our preferred cash-pay discount schedule are billed to you upfront by our practice. You will incur no out-of-pocket charges at the draw center for prepaid “Client Bill” orders.
Cancellation & Refund Policy: Appointments cancelled at least 24 hours in advance are eligible for a full refund or rescheduling credit. Cancellations made with less than 24 hours’ notice or missed appointments (“no-shows”) may incur a non-refundable service fee.
- Electronic Prescribing & Controlled Substances
Prescription Policy: E-prescribing workflows are conducted securely through integrated platforms (ScriptSure). Prescriptions are issued solely at the clinical discretion of the provider following a thorough virtual evaluation.
Controlled Substances Warning: In accordance with Florida Statute § 456.47 and federal laws, Clarion Family Health PLLC strictly restricts prescribing controlled substances via telemedicine. Schedule II controlled substances (e.g., opioids, stimulants such as Adderall) will NOT be prescribed virtually. All prescribing data for Schedule III–V medications is audited against E-FORCSE (Florida’s Prescription Drug Monitoring Program).
- Website Educational Content Disclaimer
All material published on clarion-family-health.com—including educational articles, health guides, and service descriptions—is provided for informational and educational purposes only.
Browsing our website or reading public educational content does not establish a clinician-patient relationship. A formal clinician-patient relationship is established only after you complete our onboarding intake forms, accept our practice consents, and participate in a scheduled clinical consultation.
- Patient Portal Use & Account Security
Account Access: You are responsible for maintaining the confidentiality of your PracticeQ Patient Portal login ID and password.
Data Accuracy: You agree to provide accurate, current, and complete medical history, contact details, and identification during intake.
Communication Rules: Portal messaging is for non-urgent clinical follow-ups, appointment coordination, and administrative questions only. Do not use portal messaging for medical emergencies.
